Being a responsible Saluki includes taking measures to protect yourself and others from COVID-19 by making every effort to prevent the spread of this disease.  As we approach the fall 2021 semester, we continue to strongly encourage students, faculty and staff to get vaccinated and to require masks and social distancing for people who are not fully vaccinated.

Students who are enrolled in fall courses on the SIU campus, who are fully vaccinated and who upload their COVID-19 vaccination information or provide proof of vaccination on the Student Health Center portal will automatically receive a $5 gift card to Starbucks in the Student Center. To pick up your $5 Starbucks gift card, please upload your vaccination information (view instructions), wait 24 hours and visit one of the locations listed below to pick up your gift card.

Starting August 16, gift cards can be picked up Monday - Friday 11am-3pm at the following locations:

  • Dean of Students Office, 486 Student Services Building
  • Saluki Cares Office, 491 Student Services Building
  • Student Center, ID Office, 2nd floor of the Student Center
  • Student Recreation Center, Reception Desk
  • Student Health Center, Wellness Front Desk

10 Day Vaccination Incentive Drawing: 

Students who have provided proof of vaccination prior to the tenth day of classes (Friday, August 27, 2021) will be entered to win one of the following incentives:

  • $250 book voucher for the University bookstore (Awarded up to 100 students).
  • $50 Debit Dawg dollars added to the SIU ID card (Awarded up to 50 students).
  • $1000 voucher added to the Bursar bill (Awarded up to 50 students).

Winners will be notified via their @siu.edu email address the week of August 30, 2021.

8 Week Vaccination Incentive Drawing:

Students who have provided proof of vaccination prior to the 8th week of classes (Sunday, October 3, 2021) will be entered to win one of the following incentives:

  • $50 Debit Dawg dollars added to the SIU ID card (Awarded up to 50 students).
  • $1000 voucher added to the Bursar bill (Awarded up to 50 students).

Winners will be notified via their @siu.edu email address the week of October 4, 2021.

15 Week Vaccination Incentive Drawing:

Students who have provided proof of vaccination prior to the 15th week of classes (Sunday, November 28, 2021) will be entered to win one of the following incentives:

  • $50 Debit Dawg dollars added to the SIU ID card (Awarded up to 50 students).
  • $1000 voucher added to the Bursar bill (Awarded up to 50 students).

Winners will be notified via their @siu.edu email address the week of November 29, 2021.

  • If my name is drawn will I get to select my prize?
    • Names will be drawn for specific incentives.
  • What does “fully vaccinated” mean?
    • People are considered fully vaccinated:
      • 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines; or
      • 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.
  • If I was fully vaccinated prior to the incentive plan being offered, am I eligible to receive the incentive?
    • Yes, if you meet all the other criteria.
  • Who will see my form that I upload?
    • The forms will be reviewed by the Student Health Center. The Student Health Center staff are trained on COVID-19 protocols and medical confidentiality standards.
  • I lost the card documenting my COVID vaccination. What do I do?
    • Contact the organization that gave immunization to obtain proof of vaccination.

Vaccination Incentive Terms and Conditions

By uploading your COVID-19 vaccination documentation for the purpose of participating in the Southern Illinois University Carbondale vaccination incentive you hereby agree to, understand, and represent the following:

  • Participating in the incentive program is completely voluntary
  • To be eligible for a vaccination incentive, you must upload documentation confirming that you are fully vaccinated and have received a COVID vaccine recognized by the U.S. Centers for Disease Control and Prevention. International students must be fully vaccinated and have received a vaccination recognized on the World Health Organization list of COVID vaccines that have met the necessary criteria for safety and efficacy
  • Total of all incentives may not exceed the cost of attendance
  • The $1000 voucher will go towards any outstanding account balance and can be applied for the spring 2022 semester
  • Any voucher award may impact any financial aid you receive, and you understand that you should consult with financial aid prior to accepting the voucher
  • You may refuse to accept a voucher, in which event the voucher you were awarded will be provided to another eligible student
  • A voucher may be taxable, and you are responsible for any payment of any taxes
  • Southern Illinois University’s Student Health Services will maintain the confidentiality of your vaccine documentation as part of your patient file and consistent with all applicable state and federal privacy protections
  • You authorize Student Health Services to verify the accuracy of your vaccine status using governmental immunization databases
  • You must be a student enrolled on the Carbondale campus for at least one credit hour for the fall 2021 academic semester
  • You may not convert a voucher award to cash or to any other thing of value
  • You may only win one COVID-19 Vaccination Incentive (besides receiving the Starbuck’s gift card)
  • SIU has not made any promises or commitments to you to secure your participation in the incentive, nor have you been threatened or coerced into participation
  • Employees of Southern Illinois University Carbondale are not eligible for the COVID-19 Student Vaccination Incentive program. Employees are those people who currently hold a faculty, administrative/professional staff, or civil service staff appointment. Student workers are eligible.
